This is an original educational screening questionnaire, not a validated diagnostic instrument. A positive result means that a professional evaluation may be appropriate; it does not mean that you have bipolar disorder. During that period, did you experience hallucinations, strongly fixed beliefs others considered unrealistic, or a loss of contact with reality?
Have you ever had thoughts of suicide, made a suicide plan, or felt unable to keep yourself safe?
